VPHS Aquatics Athlete Clearance for the 2025-26 school year
Please go to www.athleticclearance.com to sign all the online forms and to download a physical. A medical doctor completed physical must be uploaded to the system. Once all Information is received your account will be reviewed and then approved.

    2026 Girls Water Polo Banquet RSVP Form

    💰 Cost:  $65 per additional guest (no payment needed for athlete)
    💳 Payment Options: Check made payable to VPHS AEP, Cash or Credit Card. Checks can be sent to Coach Carcich or all payments may be received at the door. Please RSVP no later than February 10th.
